Attention Deficit Hyperactivity Disorder (ADHD)

Attention-Deficit Hyperactivity Disorder (ADHD) යනු වර්තමානයේ බොහෝ ලෙස ඇසෙන සුලබ Behavioral disorder එකකි. එවැනි අයෙකුට යම් කාර්යයකට,

  • අවධානය දීමේ අපහසුතාවය
  • අධික ක්‍රියාකාරීත්වය
  • ආවේගශීලිව හැසිරීම වැනි

ඉහත සදහන් මූලික ලක්ෂණ පැවතිය හැක. බොහෝදෙනා සිතන්නේ ADHD කුඩා ලමයින්ට පමණක් සීමාවු තත්ත්වයක් ලෙසයි, නමුත් එය වැඩිහිටියන්ටද පැවතිය හැක. බොහෝවිට සිදුවන්නේ එය ලමයාගේ කුඩා කාලයේදී ආරම්භවී ඔවුන් වැඩිහිටියන්වු පසුවද පැවතීමයි.

වයස අවුරුදු 9 -12 දී ADHD ඇති ලමයින් සාමාන්‍යයෙන් හදුනා ගැනීම සිදුවේ. ADHD හදුනා ගැනීම අපහසු වන්නේ ADHD සමග Autism වැනි වෙනත් තත්ත්වයන් පැවතින විටයි. බොහෝ දෙනා ADHD නමේ ඇති තේරුම වැරදි ලෙස අර්ථකථනය කර ඇත. එනම් සෑම ADHD ඇති පුද්ගලයන්ටම වාඩිවී සිටීම හා අවධානය දීමේ හැකියාව අඩු බවයි. එය වැරදි නිගමනයකි. එම ලමයින්ගේ අවධානය (attention) වර්ධනය වු විට ඔවුන්ට සාමාන්‍ය ලමයින් මෙන් ඉගෙනීමට හැක.

  1. ආවේගශීලිව හැසිරීම (Impulsiveness)
  2. අධි ක්‍රියාකාරීත්වය/නොසන්සුන් කම (Excessive activity or restlessness)
  3. කාලය කලමනාකරණය කල නොහැකිවීම ( Poor time management skills)
  4. සැලසුම් කිරීමේ හැකියාව අඩුකම ( Poor planning)
  5. ඉතා ඉක්මනින් යම් දෙයකට ඇති අපේක්ෂා බිදවැටීම (Low frustration tolerance)
  6. ඉතා ඉක්මනින් මනෝභාවය වෙනස් වීම (Frequent mood swings)
  7. ඉක්මනින් කේන්තියාම (Hot temper)
  8. සංවිධානාත්මක බව අඩුවීම (Poor organisation skills)
  9. ආතතිය පාලනය කිරීමට නොහැකිවීම (Trouble coping with stress).
  • එක් ස්ථානයක වාඩිවී යම් කාර්යයක් කිරීමට අපහසු වීම
  • තම වාරය ලැබෙන තුරු බලාසිටීමට නොහැකිවීම
  • නිතර පඩිපෙළ නැගීම, බැසීම
  • ජනෙල්වල එල්ලීම වැනි ලක්ශණ පෙන්නුම් කල හැක.

ඉහත ලක්ෂණවල තීව්‍රතාවය පුද්ගලයාගෙන් පුද්ගලයාට වෙනස් විය හැක. මේ හේතුවෙන් එම පුද්ගලයන්ට ඔවුන්ගේ එදිනෙදා කටයුතු වලට, අධ්‍යාපනයට, රැකියාව කිරීමට බාධා ඇතිකරයි. අන්අය සමග ඇති සම්බන්ධතා පලුදුවීම්, ආත්මවිශ්වාසය බිදවැටීම් සිදුවිය හැක.

ඉහත හේතු නිසා ADHD ඉක්මනින් හදුනා ගැනීම වැදගත්වේ. මේ සදහා සෑම විටම විශේෂ උපදෙස් පතන්න.

ADHD සමග ඇතිවිය හැකි අනෙක් රෝගී තත්වයන්…

  1. මානසික ආබාධ (Mood disorders)

ADHD නිසා නිතර නිතර තම කාර්යයන් අසාර්ථකවීම හෝ තම කාර්යයන් කරගැනීමට නොහැකි වීමෙන් ඇතිවන කලකිරීමෙන් මානසික පීඩනයක් බවට පත්විය හැක.

2. Learning disability

ADHD හේතුවෙන් ඔවුන්ගේ වයසට අදාල බුද්ධියට හා අධ්‍යාපන ක්‍රියාවලියට බාධා විය හැක. Learning disability හේතුවෙන් එම පුද්ගලයන්ට කථා කිරීමේ ප්‍රමාදයන් හා තේරුම්ගැනීමේ හැකියාවන් දුර්වල විය හැක.

This is a story of a professor at Colorado State University and well-known speaker on autism and animal handling on different platforms such as TED Talk. She overcame the limitations imposed on her by autism such as the delay in speech until age four and the inability in dealing with people. However, she was successful in getting a PhD and becoming an expert in animal husbandry.

An engaging movie to keep you entertained while getting an insight into autism. Forrest’s character depicts autism related features , however he manages to stand as an exemplary figure to the world.

A story of an Indian with autism, who embarks on a journey to give a message to the president of the United states of America.

A Brilliant Young Mind (2015)

This movie is also known as X+Y. A socially awkward teenage math prodigy finds new confidence and new friendships when he lands a spot on the British squad at the International Mathematics Olympiad.

මෙය ජාන විකෘතිතාවයක් හේතුවෙන් ඇතිවන රෝගී තත්ත්වයකි. මෙම රෝගී තත්ත්වය ට්‍රයිසෝම් 21 ලෙසද හදුන්වනවා. මෙහිදී 21 වන වර්ණ දේහය වෙනස් වීමකට ලක්වේ. මිනිස් සිරුරේ වර්ණදේහ 46 පවතී. ඒවා වර්ණ දේහය යුගල් 23 ලෙස පවතී. වර්ණදේහ මගින් පරම්පරාවෙන් පරම්පරාවට ලක්ශණ රැගෙනයාම සිදුකරයි. මෙම ක්‍රියාවලියේදී මවගෙන් වර්ණදේහ 23 හා පියාගෙන් වර්ණදේහ 23 දරුවාට ලැබීම සිදුවේ. මෙහිදී 21 වන වර්ණදේහ අතිරේකව ලැබීම සිදුවේ මෙය Down syndrome තත්ත්වයට හේතුව වේ.

මෙහිදී කථන චිකිත්සාවේ (speech therapy) වැදගත්කම නිතර මතුවේ. ඩවුන් සින්ඩ්‍රොම් සහිතපුද්ගලයන් තම අදහස් ප්‍රකාශ කිරීමට අනිවාර්යයෙන්ම පුරුදු කලයුතුය. එසේ නොමැතිවිට අනාගතය ඔවුන් තම කාර්යය ස්වාධීනව කල හැකිද යන්න දෙමාපියන් වශයෙන් ඔබ සිතිය යුතුමය. කුඩා කලසිට කථන චිකිත්සාව ලබාදීම එම නිසා සුදුසුය.

ඔවුන් අනෙක් කුසලාතා සමග කථන හා භාෂා හැකියාව වර්ධනය කල යුතුවේ. ඩවුන්සි න්ඩ්‍රොම් සහිත ලමුන් සමාජශීලිය, නැටුමට, ගැයුමට දක්ශය. කථන චිකිත්සාව සමග භතික චිකිත්සාව(physio therapy) , වෘත්තීය චිකිත්සාව (Vocational therapy) ආරම්භ කිරීම සුදුසුය.

එවිට ඔවුන්ට ඔවුන්ගේ ජීවිතය තනිවම පවත්වගෙන යා හැක. මේ සදහා සියලු දෙනා ඔවුන්ට උදව් කරමු.

Stroke and its affect on speech

Stroke occurs when part of the brain losses its blood supply. Depend of the lesion that effect results can be change. A person with high blood pressure are more prone to have strokes. Also head injury also leads to stroke.

All those scenarios will damage the brain, which is the central unit of the body and is in charge of different functions

In transient Ischaemic attacks or mini stroke you will get the warning signs. Its a medical emergency take necessary medical actions

Early Warning Signs of a Stroke

Always remember F.A.S.T so you can save someone life

  1. Face drooping
  2. Arm weakness
  3. Speech difficulty
  4. Time to Call 1990/911

As a Speech Language Pathologist, I have witnessed many patients with stroke/ brain damages that has ultimately affected their speech function. These are conditions such as Aphasia, Dysarthria..etc

In all these conditions they will have some sort of speech difficulty along with poor muscle strength. Some patients will,

  • Answer totally unnecessary things when we ask something but they can understand what we asked.
  •  Few are unable to understand what we asked.
  •  Difficulty in naming,
  •  Difficulty in repeating
  •  Slurred speech
  •  Poor speech clarity
  •  Inability to speak in mother tongue but good at secondary languages….

When brain is damaged it is difficult to function as before because it damages the neural system. Depending on the location and the severity of the damage, the symptoms will also be different from person to person.

Changes after stroke


1) Physical changes
2) Cognitive changes
3) Behavioral changes

1) Physical changes can be noted very easily


Few may have paralysis in one part of the body due to stroke. Others will have poor muscle strength which leads to difficulty in performing day to day activities as before. When it affects the speech muscles, the speech will be unclear,slurred…

Few of them have hearing and visual deficits depending on the location which the brain injury occurs. Spasticity and tremors can also be seen with them.

Apart from that they will have fatigue and weakness so they won’t be able to preform as before and they get tired easily.

If brain damage leads to paralysis then their mobility will be affected, so they may need wheelchairs or some sort of support to move from one place to another.

2) Cognitive changes


Most of them will have memory issues because damage to brain will affect the memory.
They will be having issues related to taking decisions, thinking, planning and sequencing an event , all due to the lesions which are damaged in the brain.

3) Behavioral changes


All above facts will affect the person to a significant extent. He/she was independent before the brain injury and in this new situation they are totally/partially dependent on others. This makes a huge impact on the person and it belittles their self-esteem. He/she may feel they are not worthy anymore…

Due to all those changes he/she will try to avoid social gatherings. In a situation like this all the family members need to show serious care for the patient and the emotions of them.

When they feel they are all alone, they will be frustrated and might show anxiety behaviors on the family members so try to talk to them or play some board games with them. Don’t treat them as a child because that will make them more angry.

All these matters need to be sorted out in a good manner. Because the patient was a normal human and all sudden he/she has become totally dependent in this situation.

Communication plays a big role with a person who is suffering from brain injury because it is the main bridge to express his/her needs.

Start speech therapy as early as possible for speedy recovery. There are moments in our life where there is only a small time window, so use it effectively...

වැඩි ආදරය හා කථන ප්‍රමාදය (Delayed speech)

Delayed speech (කතාකිරීම ප්‍රමාදය) වීමට දරුවන්ට ලැබෙන වැඩි ආදරය බලපායි. කථන චිකිත්සාවට (Speech therapy) පැමිණෙන බොහෝ දරුවන්ට මෙම ප්රශ්න ඇත. දෙමාපියන් වශයෙන් ඔබ තමන්ගේ දරුවාට, මුනුබුරා/මිනිබිරියට ඉතාමත් ආදරය බව අපි දනිමු. එහෙත් ඔවුන්ගේ කතාකිරීමට සැලැස්වීමට ඔබගේ දායකත්වය අවශ්‍යයි.

දරුවාගේ දෙමාපියන් රැකියාවකට යාම නිසා බොහෝවිට වැඩිහිටියන්ට දරුවා බලාගැනීමට සිදුවේ.බොහෝ වේලාවට ඔබ ආච්චි/සීයා වශයෙන් මුනුබුරා/මිනිබිරිය සමග දහවල් කාලයේදී වැඩි වේලාවක් ගතකරයි. මෙය දෙමාපියන්ට මහත් ශක්තියකි. මෙයින් දරුවාගේ පවුලේ බැදීම වර්ධනයවේ. දෙමාපියන්ට අමතරව වෙනත් අය සමග ගනුදෙනු කිරීමට හුරු වේ.

ඔබ බොහොවිට කරන්නේ ලමයා ආස කාටුන්/සින්දු පෙන්වීමයි. ඔබේ මුලික අරමුණ වන්නේ ලමයා අඩන්නේ නැතිව තබාගැනීමයි.
මෙය හොඳ ආරම්භයක් නොවේ. ලමයා මෙම ක්‍රමයට හුරුවේ. පසුව ලමයා සැම දෙයටම ඇඩීමට පුරුදු වේ. කතාකිරීමෙ අවශ්යතා ප්‍රකාශ කිරීම අඩුවේ.

පසුව දරුවාගේ දෙමාපියන් යමක් පැවසුවද ලමයා ඇඩීම සිදුකරයි. දෙමාපියන් යමක් නොදුන් විට ලමයා ආච්ච්/සීයා ගෙන් එය ලබාගනී. මෙය ඒ තත්වය දක්වා වර්ධනය වේ. මෙයින් ලමයාගේ කථනය ප්‍රමාදයවේ (Delayed speech).

අපි ලමයාගේ කථන පරීක්ෂණයට (speech therapy assessment) ලමයාගේ ආච්චි /සීයාද පැමිණීම අගය කරන්නේ මේ හේතුව
නිසායි. එවිට අපට පහසුවෙන් මේ පිළිබඳව ඔවුන්ව දැනුවත් කල හැක. දෙමාපියන් හා වැඩිහිටියන් මෙම තත්ත්වය තේරුම්ගෙන ලමයා තම අවශ්යතාව ප්රකාශ කරන තෙක් එය ලබා නොදිය යුතුය. මෙවැනි එකගතාවය දෙමාපියන් හා ආච්චි /සීයා ඇතිකර
ගතයුතුය.

කථන ප්‍රමාදය (Delayed speech) වලකාලීමට ආච්චි/සීයාට කල හැකි ක්‍රියාකාරකම්.

1) මුනුබුරා/මිනිබිරියට පැරණි කතා කියාදී .ඒවයෙන් සරල ප්රශ්න ඇසීම.
2) නැවත කතාව කියන ලෙස දරුවාට කීම.
3) සෙල්ලම් ගෙදර ඔවුන් සමග සිටීම. මෙහිදී ලමයා තනිව සෙල්ලම්බඩු සමග කතාකිරීම සිදුකරයි. හැකි සෑම විටම එයට අදාල ප්රශ්න අසන්න.
4) පැරණි Photos මුනුබුරා/මිනිබිරියට පෙන්වා ඒ ගැන විස්තර කරන්න.
5) සරල ක්‍රිඩා කියාදීම.
6) දරුවන් නිවසේ කටයුතු වලට සම්බන්ධ කරගැනීම. උදාහරණ ලෙස ගෙවතු වගාවට, නිවස පිරිසුදු කිරීම, ආහාර පිලියෙළ
කිරීමට….

දෙමාපියන් ඔබේ දරුවා වෙනුවෙන් කාලය වෙන්කර ගන්න. මෙම කාලය නැවත ලැබෙන්නේ නැත. ඒ කාලය හැකි සෑම විටම දරුවා සමග කතා කිරීමට, සෙල්ලම් කිරීමට සිදුකරන්න. එවිට දරුවාගේ කථන වර්ධනය වීමක්ද සිදුවේ. නිවසේදී කල හැකි සරල speech therapy activities කිහිපයක් අපි ඔබට ලබා දී ඇත. ඒවා කියවා එම ක්රියාකාරකම් නිවසේදී සිදුකරන්න...

Speech development at home

Have you noticed a change in speech development at home with the current trends of many people moving into apartments? Try reading this post 10 years from now, and you will still agree with me.

Mark my words, this piece of advise will help you and your family in years to come. As adults we remember what we did in our childhood on weekends, after school or during school vacations. We used to visit our cousins, stay over at their place, play a game of cricket with other children or ride a bike to show it off to our friends, collect different cards with friends, etc …

Now think about you or child. How many of you allow him/her to play with your neighboring kids?

At our households, it was necessary to play an active role as a family member. The family meant a lot to us. So our parents always gave us a chance to help each other. We learnt many new skills,told many stories at the family dinner table,etc. There was room for speech development at home.

If you take a look at the Colombo skyline you will realise that it is changing drastically.

Nowadays, many parents in Colombo move in to these apartments with their kids. They do not know the pros and cons of how it will affect their child. Have you ever wondered where they are going to play? Rooftop garden? Do you really allow them to play or do you think it’s too risky?

Pros & Cons on your child’s communication

I have met parents who live in apartments and in single storied houses. As a Speech Language Pathologist is not my duty to take decisions about your life. I don’t mind where you live, but I do care about your child’s Speech and Language development.

Most of the parents realize the truth of it only when we speak to them.They realize the truth of it and say, ” We used to play with other kids and we had a really great time back then.But our kids don’t have that opportunity”.

About speech development

This happens mostly because parents are really worried about the society. We cannot blame them because on a daily basis you hear shocking news. This ripple effect is taking place in many other countries too. Studies on this area suggests that there is a huge risk in parents isolating their children with phones,etc.

That doesn’t mean that we should forget the childhood activities we used to do. We still need to give our children a play time.It should be a time for interaction with peer groups just like what we had when we were kids.

Sometimes you find kids going to the same school/play school but they won’t interact when they are in the apartment. Do you remember how you went to your friends home (who lived 2,3 lanes apart from yours), and asked for permission from their parents to go to the ground to play?

It was very enjoyable for us back then. But does your children know how great it is to unite and play?


Do you realise that your children are not used to going out. To be frank, we don’t find many play grounds like those days. That’s because we are all caught up in a rat race where children are trained to play inside the house. They are not allowed to mingle with the society until they reach a certain age group.

You need to focus more on your kids. Let them have good social interaction and communicate with other kids. Speech development at home is necessary. It will eventually help them to work on their Speech and Language skills.

ඔටිසම් තත්වය ඇතිවීමට හේතු බොහෝ ප්‍රමාණයක් තිබිය හැකිය. නමුත් එය නිශ්චිතවම කුමක්ද යන්න කිව නොහැකිය. වර්තමානයේ නව ප්‍රබේදයක ඔටිසම් තත්ත්වයක් හදුනාගෙන ඇත. එනම් Virtual autism, මෙයට ප්‍රධාන හේතුව වන්නේ අධිකව TV, Phone, Tabs නැරඹීමයි.

ඔටිසම් සහිත ලමයකුගේ ලක්ශණ ලමයාගේන් ලමයාට වෙනස් විය හැකි සහ රෝගයේ තිවුරතාවය(Severity) වෙනස්විය හැක.

Play to develop speech

Today we are going to tell you the importance of play to develop children with speech delays. As speech & language pathologists we always recommend your children to play with their bothers & sisters or with you (parents). Unfortunately it is not happening in the current society. Most of the parents are busy with work so the kid will miss their play time. In situations like this i would recommend them to send their kid to a play school along with regular speech therapy.

Don’t get me wrong, all we are trying to do is giving your child another chance to interact with others. This should not be the best in town. You can find a place near your home and get the child to mingle with new faces and interact with them on a daily basis.

Because a play school will be the ideal platform where you can find a peer group for your child.

Nowadays most of the families have one or two kids. So activities involved with play and social interaction is very limited.This is why it is necessary to know the benefits of play to develop speech.

How play school will be helpful while attending speech therapy

When a child attends play school, the child will develop his/her pragmatic skills and during speech therapy they will develop their speech and other language skills. These need to go hand in hand. So make sure your child gets enough play and Speech and Language stimulation at the same time.

Also, even after we send the child for play school still there is a tendency they could be idling (sit alone) and playing alone. Talk to the teacher and plan a daily routine where the child gets a chance to engage with others. Do not forget the importance of play to develop speech.

As a parent communicate with both teacher and Speech Therapists and update them on the goals that the Speech Therapist is working on. That will be a great help for everyone and especially your child.

Meanwhile don’t forget that the Speech therapists will be working for their Speech and Language goals. As a result your child will develop miraculously which will relieve you from your parental pressure and concerns with regard to child’s growth and development.

Don’t allow COVID-19 to affect your child’s speech and language

By this time we all know that speech and language therapy centers, nurseries and schools are closed due to the outbreak of the current COVID-19 epidemic. The severity of this pandemic is as such that even the president of United States – Donald Trump has also become a victim. Many of us are shocked, others are afraid and the rest are ignorant…

While a lot of things are taking place, our main duty is to protect the younger generation. So parents are keeping their children indoors. However, this has its pros’ and cons’. Children are happy because they don’t have to work or be under the supervision of a teacher / speech & language therapist.

But as speech & language therapists we know the effect it will have on your child by not being able to attend regular therapy sessions. Imagine if the child was not sitting but has just begun to sit and work with the therapist. It’s really hard to make a child sit or speak his first meaningful word and it is one of the main goals of the speech & language therapists.

We request all our parents to work on the speech therapy activities as much as you can at home or attend online speech therapy and keep your children engaged during these critical days before the kids droop down to the same level they were in ,before attending speech therapy.

So let’s see what techniques of speech and language therapy we can use at home to get your child to be on track during COVID-19,

1) Make a time table

This is the best way to start and if not now then when? So make sure to follow a time table from the start. Keep the timetable in a place where you and your child can always see. This will be a good strategy to put into practise if you want your child to balance both work and play. If you can allocated a room/space for your child to do these activities.

2) Reduce TV, phone, tab as much as you can and keep them busy with speech stimulating methods

Make use of your holiday to get to know your child better. Spend time with them and talk to them rather than sitting infront of a screen. On the other hand do not let your child take charge of the TV remote or let them unlock the phone without your permission.

3) Engage them in more of physical activities during this endemic

This is the best time to go out and be in the sun, Play with sand and learn to write letters, Teach about nature to your child, Scream while playing run and catches, Play cricket and tell your child to count the no. of balls,Speak loud and clear while you do hide and seek with them, Sing songs, Engage in creative arts and crafts, etc.

4) Sit and tell stories that they enjoy

Bring back the past. Tell stories that your grandparents taught you.. tell them about the era you lived in.. how you used fall and hurt yourself.. and how everyone gathered and played imaginery play house,etc.

5) Talk to speech therapists and try to work on those speech and language therapy goals during COVID-19

As developing a child is a joint effort, even if they are on leave.. speech therapists will be able to assist your child. Ask them for more tips to work on at home and follow them closely.

6) Ask frequent questions to improve their speech and language

While you do all the above make sure you converse with them and ask more questions, explain it to them and get them to ask questions back. Let it work both the ways.

A must read before attending a speech assessment

These days there is a growth in the number of parents who consult a speech therapist/speech & language pathologist for their child or a loved one. But have you ever wondered what you should know before attending a speech assessment?

If not, this post is dedicated to all of you out there who are ignorant of the process involved when taking your child/ an adult for speech therapy. So let us start….

I am sure by this time you know who is a speech therapist, but if you do not know we will tell you. A speech & language pathologist /speech therapist is an amazing professional , who deals with any sort of speech, language or swallowing difficulties.

For example they will help to improve communication skills of children with Autism, ADHD, Delayed Speech, Apraxia of Speech, Down syndrome, Cerebal Palsy or adults who have Neurological conditions such as Aphasia, Dysarthriya… or ones who have voice problems or Fluency disorders such as stuttering/stammering…You name it!

During the initial assessment, your speech therapist/pathologist will ask information in detail about your child/ adults communication abilities. It is also known as taking the case history of the person. It is kind of an interview with a few questions, observation and sometimes they will use different test materials to get more details.

Here I am going talk about 5 things you must do before the speech assessment

1) Please be on time

Time is gold. So please make sure to be there on time for your speech assessment.

2) Bring all medical records

On your visit make sure to get all your child’s , his/her medical reports with you. This will help the speech therapist to find out, if there is any cause behind the problem.

3) Tell your expectations

When the speech therapist asks ” What is the reason you visited us today?” Please share your thoughts or why you have come to meet them and express your issues…so speech therapists will have a clear idea.

4) List it down before the speech assessment

If you can write down all the words he/she speaks. (If your child does not speak any word that is fine) Also make a note of what he/she can understand (Does not necessarily need to talk). The reason why I am telling you to write these is because then you would not miss any information during the assessment. If you are not sure of some answers please tell that you are not sure, so the speech therapist will make a note and check on it during the first therapy session. If he /she is attending school/ nursery bring their books with you. Those gives an idea of their academic skills.

5) Take an action

After the assessment , speech therapist will tell you what you need to do next. Make sure you clear all your doubts before you leave. What ever they recommend please fulfill it, because at the end it will help your child. If they recommend you to start speech therapy make sure you start speech therapy as soon possible.

Because by looking at it as a speech language therapist we can understand the current communication situation or speech and language abilities of the child. First, those information will serve as a baseline and we as professionals will build the rest of their communication abilities depending on that.
